- Online Submission:

- Last two annual marks sheets (duly attested).
- Parents’ or guardians’ original income certificate.
- Attested school progress report/marks sheet from the last annual examination.
- CNIC number or NADRA B-form (for students under 18).
- Certification:

- Forms prepared on plain paper or printed forms afresh will not be accepted.
- Head of Institute’s Role:
